The OTES PTA is committed to ensuring that all students have the opportunity to participate in our After-School Enrichment Activities, and we will offer financial aid to qualified families.
NEW: Starting Winter 2017, aid will cover up to 50 percent (half) of the cost for an eligible student. Students who require more significant assistance should contact email@example.com.
Aid is available only for PTA-sponsored extracurricular activities. This does not include KAH, Girls on the Run, Let Me Run, etc.
Our pool of funds is limited, so we ask that you apply only if you have a significant financial need and your child would otherwise be unable to participate in the enrichment activity. Assistance are subject to the following eligibility conditions and restrictions:
- Participating parents should select the level of scholarship on the form. Parents are responsible for the remaining tuition.
- Up to two scholarships per child per school year. With the exception of year-long programs (FLES), parents must apply separately for each semester’s activity. Yearlong programs are eligible but will be considered to complete the scholarship availability for that student for the full year.
- The student must actually attend the enrichment activity on a regular basis. Failure to do so will result in the student being ineligible for future scholarships.
- A parent or guardian must be a current member of the PTA.
To be considered for aid, parents must complete an application form (below) and attest to a financial need. All personal information submitted in connection with an application (including the fact that a particular student is requesting or receiving aid) is confidential, except as necessary to pay fees and report aggregate information to the PTA.
By signing below, you attest that it would place a financial hardship on your family to pay full tuition for your child to participate in a PTA-sponsored after-school program.
